1、自由销售证明书一般格式要求

自由销售证明书并没有固定的格式,一般要求如下:①抬头需有公司名称地址;②文件名称一般为“CERTIFICATE OF FREE SALE”;③文件内容一般要涉及到出口商、进口商、产品名称等,体现“可自由销售”、“对人体无害”、“适合消费”等内容;④公司在落款处盖公章或公司签章,不可以盖单证章。

2、申办时提供由专门主管部门出具的产品生产许可证或卫生许可证或产品合格证,如果申请人不是生产商,还须提供申请人和生产商的购销合同,并提供生产商的营业执照;
